For a statement you want to find again. Record the page, the visible date, the relevant wording and what remains unclear. Includes an invented example showing how an interpretation can exceed the original sentence.
For a point you want someone to explain. Separate the reference from your question and leave an honest space for an unknown answer. The format is a suggestion for your own paper or document, not a form collected by this site.

The blog article follows the same note through several edits: selecting a purpose, finding the source, shortening the sentence and making the missing context visible. It explains why an unanswered question should stay unanswered until the appropriate person can address it.
