What is published here
Health Reading Notes provides educational material about reading sources and organising questions. Its examples show ways to separate a source statement, an interpretation and an uncertainty. They are editorial writing exercises, not medical tests, treatment plans, professional opinions about a particular person or validated assessment tools.
You may read the pages without an account or payment. The current site offers no purchase, subscription, appointment or consultation. It cannot receive a patient history or respond to an individual request for medical guidance. Reading a page does not create a clinical relationship.
A historical name is not a current attribution
The domain previously hosted a profile headed Dr. Manish Nigam. The history page identifies the specific dated source and explains the limited observations drawn from it. Those observations do not confirm current credentials, clinical arrangements, availability, contact information or affiliation.
The present publication is independent. Its new writing is not authored, medically reviewed or endorsed by the historical professional. A publication byline identifies the current editorial material; it must not be interpreted as a claim of medical qualification or a professional review that did not occur.
General information and personal decisions
No page on this site can assess your circumstances. Do not use a writing exercise to determine a diagnosis, decide whether a concern is urgent, or start, stop or change treatment. A complete-looking note is not evidence that a health issue has been evaluated. Seek appropriate professional assistance for individual health decisions, and do not postpone needed care while trying to finish an exercise.
Examples are deliberately bounded. When a scenario is marked fictional, its characters, wording and situation are illustrations. They are not patient testimonials, clinical studies or claims that a method produced a particular outcome. The publication does not promise that its format will improve health, secure a particular appointment result or answer every question.
Sources and external destinations
Links identify the material supporting a specific point. A source’s organisation does not thereby sponsor, endorse or medically review this publication. Sources can change after the date a page is written. The article’s explanation and the external page remain separate documents, each with its own scope.
General source-reading guidance from a United States institution is not presented as local healthcare-access advice or a statement of law in another country. Follow the actual scope of the cited material. If an external service asks for data or offers a transaction, its own terms and practices apply to that interaction.
Using notes and revisiting pages
Any note you make is kept outside this publication. You decide what to record and where to store it. Do not assume a private communication channel exists because an article suggests preparing a question. The privacy page describes the static site’s actual configuration.
Page dates identify the edition of current editorial material. A historical date identifies a past source, not a continuing service promise. If a statement is unclear, return to the linked evidence and preserve the uncertainty rather than replacing it with an assumption.
